You acknowledge telehealth services have inherent limitations, and certain conditions may not be suitable for telehealth diagnosis or treatment. Providers retain discretion regarding treatment suitability.

You consent explicitly to electronic communications, including emails, which may contain your health information.

In accordance with the federal Physician Payments Sunshine Act, you may review payments or transfers of value made to providers by pharmaceutical and medical device manufacturers through the federal Open Payments database: https://openpaymentsdata.cms.gov.

You understand and accept responsibility for all fees associated with telehealth services and medications. Silhouette MD may have financial relationships with certain pharmacies or labs. You are free to choose alternative providers or pharmacies and may request prescription transfers.

You may withdraw consent for telehealth at any time by notifying us in writing. However, such withdrawal will not affect actions already taken in reliance on your consent.

Concerns regarding medical care may be directed to your state’s medical board.
